The Suffering: Ties That Bind is a psychological survival horror game and the sequel to The Suffering. Players once again take control of Torque, who has escaped the horrors of Carnate Island and now finds himself in the streets and prisons of Baltimore.

As Torque searches for answers about his past and the mysterious man connected to his family's death, he must battle grotesque creatures while exploring abandoned buildings, prisons, underground areas, and the dangerous streets of the city. The game combines first-person and third-person combat, giving players the freedom to switch perspectives during exploration and battles.

Torque can use a variety of firearms and melee weapons, while his monstrous transformation gives him additional power against some of the game's most dangerous enemies. The morality system from the original game also returns, allowing the player's actions to influence Torque's relationships, his story, and the eventual outcome.

With its disturbing atmosphere, brutal combat, psychological horror, and dark urban setting, The Suffering: Ties That Bind expands on the original game's formula while continuing Torque's violent and unsettling journey.
